Position Overview This dual-role position plays a vital part in ensuring regulatory compliance, infection control excellence, and accurate reimbursement through high-quality MDS assessments.

You’ll collaborate closely with nursing leadership, interdisciplinary teams, and frontline staff to maintain a safe, compliant, and resident-centered environment.### ###  Infection Preventionist Responsibilities:
* Oversee and manage the facility’s Infection Prevention & Control Program
* Track, trend, and report infections in accordance with CDPH and CDC guidelines
* Conduct infection control surveillance and audits
* Provide staff education and training on infection prevention practices
* Ensure compliance with state and federal regulations
* Serve as liaison during audits and surveys### ###  MDS Nurse Responsibilities:
* Complete and coordinate accurate and timely MDS assessments
* Ensure proper documentation to support reimbursement and quality measures
* Collaborate with IDT to develop and update care plans
* Monitor case mix and quality indicators
* Maintain compliance with Medicare/Medicaid regulations### ###

Qualifications:

* Current California
** LVN license** (required)
* MDS experience preferred
* Infection Prevention experience preferred (IP certification a plus)
* Strong knowledge of skilled nursing regulations
* Detail-oriented with strong organizational skills
* Ability to work independently and collaboratively
* Committed to high-quality resident care###
