This is a full-time on-site role for a Behavioral Health Consultant at our Nevada, Missouri location. The Behavioral Health Consultant will collaborate with patients, families, and healthcare providers to support emotional and behavioral well-being. Responsibilities include conducting behavioral assessments, developing and implementing evidence-based strategies, providing individual or group interventions, and promoting wellness initiatives within the healthcare setting. The consultant will play a key role in addressing behavioral health needs and improving patient outcomes within the community.
Qualifications- Strong expertise in Applied Behavior Analysis and Behavior Analysis
- Knowledge of Psychology, Behavior Modification, and Behavioral Health principles
- Experience in assessing, developing, and implementing behavioral health plans
- Excellent communication, collaboration, and organizational skills
- Ability to work effectively in an interdisciplinary team setting
- Previous experience in a healthcare or community health setting is an asset
As the NRMC Population Health Coordinator I will add that we have two great teams in our Pop Health Department:
Primary Care Health Home (PCHH) and Chronic Care Management (CCM). These two entities focus on our Medicaid and Medicare population, to be the bridge between the patient and the provider especially in between visits to ensure medication compliance, understanding of medications, being a phone call away for medical-emotional-social needs. We have a dry pantry to assist those in need. Our patients have expressed how much they appreciate these nurses who explain things they do not understand or may have missed during the clinic visit;
assisting getting an appointment sooner than scheduled because symptoms that may have changed or they have new symptoms that require a visit. They also enjoy meeting these nurses in the clinic during a visit and they have a face to put with the person who calls and helps them.
As the BHC you would be part of this team. You will be able to help any patient who needs to understand their chronic medical issues and prognosis for those diseases; assist with building care plans for patients' health improvement and hold education opportunities for patients to learn and develop healthy practices for their diseases. There is so much more to this position.
