Team Lead Caregiver

Continuum is looking for a skilled, kind, and patient Team Lead Caregiver to support a female client in her home. The Team Lead Caregiver must be highly confident with gait belt pivot transfers, full assistance with personal care, and comfortable working with dogs in the home.
This role includes leadership responsibilities and flexibility to fill in when other caregivers are unavailable.

Three 12-hour shifts (7:00am-7:00pm) weekly + On-Call (ability to fill in should a call off occur)

• Provide total assistance with bathing, dressing, toileting, grooming, and hygiene
• Perform safe gait belt pivot transfers and mobility assistance
• Support positioning, safety monitoring, and daily routines
• Assist with meal prep, feeding if needed, light housekeeping, and medication reminders
• Communicate effectively with family and care team
• Provide companionship and maintain a calm, supportive environment
• Comfortably interact with and help care for household dogs

• Proven caregiving experience with mobility-impaired clients
• Strong skills in gait belt / pivot transfers
• Dependable, flexible, and comfortable taking additional shifts when covering call-offs
• Kind, patient, and professional
• Able to work 12-hour shifts; 7:00am-7:00pm
• Must genuinely love dogs

• Mature, dependable caregiver with strong homecare experience
• An individual who is patient, compassionate, and confident
• Works well independently and takes initiative
• A professional who understands boundaries, privacy, and person-centered care
• Able to provide and maintain a valid driver’s license, proof of insurance, and reliable transportation
• Able to pass a background screening
• Can move objects averaging 100lbs (with use of assistive devices)
• An effective communicator with both oral and written communication
