Contract Weeks: 13
Days: Mon, Tue, Wed, Thu, Fri
Responsibilities- Assess patients' physical conditions and develop individualized treatment plans.
- Provide hands‑on physical therapy to improve mobility and reduce pain.
- Document progress and outcomes to track patient improvement and generate reports.
- Set measurable rehabilitation goals aligned with patient milestones and clinic targets.
- Collaborate with physicians and multidisciplinary team to optimize care plans.
- Adhere to evidence‑based practice and continuous professional development.
- Manage patient load efficiently to maximize daily throughput while maintaining care quality.
- Educate patients and families on home exercises and injury prevention.
- Monitor and report treatment outcomes to support performance‑based advancement.
- Maintain clinical equipment and ensure a safe, compliant treatment environment.
- Provide on‑site therapist support within defined territory and participate in pipeline reviews.
- Assist in program development to expand services and grow patient referrals.
- Licensed Physical Therapist with valid state license and current CPR/First Aid.
- Experience conducting initial assessments and developing individualized treatment plans.
- Hands‑on therapeutic skills for mobility, pain reduction, and gait training.
- Commitment to evidence‑based practice and ongoing professional development.
- Proficiency in measuring outcomes and documenting progress for reports.
- Ability to set SMART rehabilitation goals aligned with clinic targets.
- Collaborative mindset to coordinate with physicians and multidisciplinary teams.
- Efficient patient load management while maintaining quality care.
- Educate patients/families on home exercise programs and injury prevention.
- Maintain equipment, ensure safety/compliance, and uphold clinic standards.
