Job Title:

Speech-Language Pathologist (SLP-CCC) Job Summary

We are seeking a dedicated and clinically skilled Speech-Language Pathologist (SLP) to join our rehabilitation team. In this role, you will be responsible for effectively screening, evaluating, and rehabilitating patients presenting with communication, cognitive, or swallowing disorders. Using evidence-based practices and standardized assessments, you will develop, adjust, and execute individualized plans of care that help patients achieve optimal functional outcomes.

Key Responsibilities
  • Clinical Assessments & Treatment: Screen and evaluate patients to identify communication, cognitive, or swallowing deficits; design and implement customized, goal-oriented therapeutic plans.
  • Progress Tracking & Adjustments: Continually assess patient responses to interventions, adjusting treatments within the clinical record as needed to optimize outcomes.
  • Consultation & Counseling: Provide meaningful education, clinical consultation, and counseling to patients, families, caregivers, and interdisciplinary healthcare team members.
  • EHR Documentation: Maintain impeccable, timely patient records within the electronic documentation system, including accurate tracking of treatment minutes in strict accordance with regulatory guidelines.
  • Discharge Planning: Deliver comprehensive, compliant discharge summaries detailing services rendered and clinical milestones achieved.
  • Collaboration & Leadership: Participate actively in interdisciplinary patient care conferences, rehabilitation team huddles, and utilization review meetings. Provide clinical supervision and mentorship to CFY clinicians or student Speech-Language Pathologists as needed.
Candidate Requirements & Qualifications Licensure & Certifications
  • Clinical Certification: Active Certificate of Clinical Competence (CCC-SLP) from ASHA [Required].
  • State Licensure: Active Speech-Language Pathologist License in the State of Missouri [Required]. (Multi‑state / Compact licenses are allowed and welcomed).
  • Professional Standards: Unwavering adherence to ASHA and state practice acts, professional codes of conduct, and ethical boundaries.
Experience & Preferences
  • Setting

    Experience:

    Minimum of 1 year of experience working in a Long‑Term Care (LTC) or Dedicated Rehabilitation setting [Required].
  • New Graduates: Welcomed and considered, provided your academic background includes comprehensive, hands‑on Level II clinical fieldwork rather than observation‑only (Level I) hours.
Core Competencies
  • Strong verbal, written, and electronic medical record (EMR) tracking skills.
  • Ability to work effectively and autonomously within an interdisciplinary rehab environment.
  • Knowledge of durable medical equipment (DME) and clinical supplies sourcing relevant to speech and dysphagia therapies.
