Provides direct care to residents, treating each with dignity and respect while promoting independence, yet meeting their individual needs in a caring atmosphere.
ESSENTIAL DUTIES/RESPONSIBILITIES
Represent Independence Care Center in a positive and professional manner.Know and practice Resident’s rights and exhibit attitudes and behaviors that indicate respect for them.Demonstrate a caring attitude in all interactions with residents.Abide by Independence Care Center Personnel Policies and Procedures as outlined in the Employee Handbook.Know and practice emergency policies and procedures.Administer nursing care to residents according to the policies and procedures of Independence Care Center.Utilize all safety equipment and precautions as per policies and procedures when transferring or ambulating residents.Must use gait belt when transferring or ambulating residents. Must use assist x2 with hoyer lift.Perform all care using the principles of standard precautions and good hand washing.Interact with all residents based upon their individual needs.Maintain a therapeutic environment for all residents; promote and encourage independence.Report to charge nurse any changes in resident’s condition.12 hours of education completed per year.All other duties as assigned.CERTIFIED NURSING ASSISTANT- CNA- SHIFT DUTIES DUTIES FOR 7:00 A.M. to 7:00 P.M. SHIFT
Receive report from off-going staff and charge nurse.Review resident’s care plan and administer care, accordingly, based on individual needs.Be aware of where assigned residents are always and their condition.Answer all call lights promptly.Assist with A.M. and P.M. care- partial baths, oral care, incontinent care or toileting, dressing.Assist all residents with breakfast, lunch, and supper. Feed residents that need assistance as assigned and document intake.Offer HS snack as indicated and assist as needed.Pass ice and water as assigned.Return those residents wishing to take an afternoon nap to bed.Assist with showers as assigned and sign shower sheet.Make rounds every two hours- perform toileting, brief changes, incontinent/pericare, ROM exercises, and repositioning of residents.Perform vital statistics/weights monthly, or as ordered, and document.Assist with transporting residents to and from scheduled activities.Perform oral care, nail care, and shaving as needed.Offer fluids throughout the shift and with rounds.Straighten resident’s rooms.Restock linen closets, shower room closets, carts, and pass fresh linens.Change linens on assigned beds as needed.Document bowel movements.Matrix Point of Care chartingPerform duties per assignment sheet and sign off as completed.Give report to on-coming staff.DUTIES FOR 7:00 P.M. to 7:00 A.M. SHIFT

- perform ADL’s, get-ups, assist residents with partial baths, incontinent care, toileting, and dressing.Perform vital statistics/weights monthly, or as ordered, and document.Assist with transporting residents to and from scheduled activities.Perform oral care, nail care, and shaving as needed.Offer fluids throughout the shift and with rounds.Straighten resident’s rooms.Restock linen closets, shower room closets, carts, and pass fresh linens.Change linens as needed.Document bowel movements.Matrix Point of Care Charting.Perform duties per assignment sheet and sign off as completed.Give report to on-coming staff.CREATIVE CARE AIDE- CNA- SHIFT DUTIES DUTIES FOR 7:00 A.M. to 7:00 P.M.
