Assist residents with activities of daily living including grooming, hygiene, dressing, nutrition, and mobility;
Monitor and report changes in residents' condition to the care team;
Take and record vital signs and intake/output as directed;
Assist with meals, transportation, and housekeeping tasks to support resident comfort and safety;
Communicate effectively with residents, families, and staff; participate in activities and promote well-being
