Adira Medical Resort is seeking a Social Worker to provide supportive services to residents and their families, including counseling and referrals to outside community or medical services. The role advocates for residents and helps organize referrals that best meet their needs.
You will work with the care team to develop discharge plans, coordinate with families, and assist with admission processes while staying current with federal and state regulations.
