Position Overview
The Oncology Pharmacistwillensuresafe ,optimalandefficaciousdrugtherapyforpatients,asyoureview,processandmonitorphysicianordersandpatientmedicationprofiles.
Asanactivememberofthehealthcareteam,theOncology
Pharmacistisresponsibleforcontactingotherhealthcareprofessionalsasappropriate,performingdutiesandresponsibilitiesinamannerconsistentwiththehospital’smissionandvalues.
- Dispenseandpreparespecializedmedicationsforpediatriconcologyandhematologypatientsandcollaborateacrossdisciplinestoshapeandexecutepatienttreatmentplans.
- Verifymedicationordersforaccuracy,appropriateness,andpotentialinteractionsandeducateandcounselpatients,families,andcaregiversonmedicationuse
- Monitordrugtherapyforeffectivenessandadverseeffects.
- Contributetopatientcarethroughactiveparticipationinclinicalrounds.
- Managechemotherapyprotocolsandsafehandlingofhazardousdrugs.
- Performmedicationreconciliationduringcaretransitionsandprovideexpertdruginformationandconsultations.
- Supportthedevelopmentofpharmacypoliciesandproceduresandensureadherencetoallregulationsandsafetystandards.
- Engageinqualityimprovementandresearchinitiatives.
- Maintainprecisepatientandmedicationrecords.
- Staycurrentwithadvancementsinpediatrichematology/oncologypharmacotherapy.
- Mentorpharmacystudentsandresidents
