As a Community Health Worker, you will serve as a trusted link between patients, healthcare providers, and community organizations. You will help individuals navigate healthcare and social service systems, identify and address barriers to care, promote health education, coordinate services, and support patients in achieving their personal health goals.
Through outreach, advocacy, and care coordination, you will empower individuals to take an active role in their healthcare journey while contributing to healthier communities across the region.

- Coordinate care and support services for individuals by connecting them with healthcare providers, behavioral health services, and community resources.
- Build meaningful relationships with patients and empower them to achieve their health and wellness goals.
- Educate individuals and families on healthcare resources, preventative care, wellness initiatives, and available support programs.
- Conduct outreach activities through phone calls, community visits, home visits, and community events to engage individuals in care and services.
- Identify barriers to care and collaborate with patients and interdisciplinary teams to develop effective solutions.
- Advocate for patients while promoting culturally responsive, person-centered care.
- Assist individuals with understanding insurance coverage options and completing eligibility and enrollment applications, including Qualified Health Plans (QHPs).
- Collaborate with healthcare providers, care teams, and community partners to improve care coordination and health outcomes.
- Maintain accurate documentation, records, and data entry in accordance with organizational requirements.
- Support departmental initiatives, quality improvement efforts, and other assigned projects.
- High School Diploma or equivalent required.
- Bachelor's degree in Health Sciences, Social Work, Human Services, Public Health, or a related field preferred.
- Two (2) years of experience in community health, patient navigation, healthcare outreach, human services, case management, or a related field preferred.
- Knowledge of community health principles, social determinants of health, harm reduction practices, and motivational interviewing techniques.
- Knowledge of local community resources, support programs, and social service systems.
- Strong customer service, relationship-building, communication, and problem-solving skills.
- Proficiency with Microsoft Office applications including Excel, Outlook, Teams, and Word.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Valid driver's license, reliable transportation, and current auto insurance required.
- Excellent interpersonal, active listening, and communication skills.
