UCP Heartland is looking for an exemplary DSP that will deliver direct services to enrolled clients in the Independent Supported Living Arrangement program and/or Pre-Independent Supported Living Arrangement, supporting individuals to live independently in the community.
This is an Hourly, FULL-TIME position.

What will you be doing in this position?
  • Responsible for implementation of all ISLA or Pre-ISLA supports as outlined in the Individual Support Plan (ISP).
  • Maintain professional relationship through regular communication with clients, families, Case Management entities, supervisor, medical professional and other service providers.
  • Maintain up to 10 hours of support for each client on caseload.
  • Support clients in understanding their health needs as it pertains to their independent living goals.
  • Support individuals in maintaining a clean, sanitary, comfortable, and welcoming environment as described in their independent living goals.
  • Actively participate as a member of a team in goal development utilizing client support plans.
  • Support clients with learning how to manage daily life activities such as grocery shopping, navigating transportation, cooking, cleaning.
  • Create weekly schedule maintaining approved working hours.
  • Complete all required documentation with details and accuracy, in accordance with guidelines established by the Agency’s EHR policy and other Regulatory Entities including but not limited to service documentation, incident reports and activity records.
  • Remain in compliance with all programs regulatory codes and requirements (i.e., Medicaid, DMH, SB40 Boards, CARF and agency policies and department procedures).
  • Maintain HIPAA compliance and confidentiality.
  • Learn about community resources to better support clients independent living.
  • Demonstrates excellent defensive driving skills, able to operate agency vehicles varying in size from a standard van to a large ADA custom van and a driving record that meets agency policy requirements.
  • Assist client with creating natural supports at home and within the community.
  • Effectively communicate orally and in writing, including promptly communicate any problems or concerns that may arise to direct supervisor.
  • Comprehension and proficiency with all agency software.
  • Secure and maintain training/certification in CPR/First Aid, CPI, Abuse and Neglect, Bloodborne Pathogens, HIPAA, and all other certifications or trainings as required by the Agency.
  • Use the EHR (Electronic Health Records) system to complete all documentation.
  • Maintain 80% billable time throughout the week.
  • Maintain a flexible work schedule to meet the needs of the clients supported.
  • Other duties as assigned.
What does UCP Heartland need from you?
  • High School diploma or its equivalent (GED).
  • Clean driving record and auto insurance
  • Experience supporting individuals with disabilities or in a customer service field.
  • Ability to take direction and work with minimal supervision.
  • Ability to consistently work in a professional, cooperative manner with all Agency clients and personnel.
  • Good written and oral communication skills
  • Must be flexible, reliable, and dependable and have the ability to multitask
  • Computer proficiency
  • VALID DRIVERS LICENSE IS REQUIRED FOR THIS POSITION
What would we prefer you have?
  • Associate degree in a human service field.
  • Two years’ experience supporting individuals with developmental and physical disabilities.
  • Knowledge of and experience with adaptive equipment and assistive technology devices for adults with developmental disabilities.
  • Knowledge of and experience with (EHR) Electronic Health Records system.
