Location: California
my Place Health, a PACE organization, seeks an MSW to plan, organize, and deliver social work services for participants in our Los Angeles center. You will maintain a caseload and collaborate with the Interdisciplinary Team to ensure person-centered care, empowering seniors to live independently at home.
