- Provide therapeutic interventions under OT supervision
- Implement treatment plans created by the Occupational Therapist
- Provide direct patient care per state and facility guidelines
- Monitor and document resident progress; report changes to the OT
- Educate residents and caregivers on adaptive techniques and equipment
- Collaborate with nursing, PT, SLP, and other healthcare staff
- Maintain accurate, timely documentation in the EMR
- Participate in care planning and discharge planning
- Ensure compliance with HIPAA, CMS, and all regulations
- Associate’s degree from an accredited OTA program
- Current NBCOT certification
- Valid state license as an Occupational Therapy Assistant
- SNF or long‑term care experience preferred
- Strong communication and interpersonal skills
- Able to work independently and as part of a team
- Committed to patient‑centered care and rehab excellence
