We are seeking a dedicated Physical Therapist to provide comprehensive evaluations, individualized treatment plans, and collaborative care that helps patients achieve their highest level of function and independence.

As a Physical Therapist, you will work closely with patients, families, physicians, and interdisciplinary care teams while upholding Mercy’s mission, values, and service standards.

Position Summary

Under the general supervision of Therapy Leadership, the Physical Therapist is responsible for performing objective patient assessments, developing and implementing individualized treatment plans, and delivering skilled physical therapy services for adult and geriatric patients. This role includes direct patient care, supervision of rehabilitation technicians, and oversight of Physical Therapist Assistants in accordance with state practice regulations.

Key Responsibilities Patient Evaluation & Care Planning
  • Perform comprehensive physical therapy evaluations following physician referrals and departmental standards.
  • Conduct objective assessments using appropriate evaluation tools and clinical judgment.
  • Establish functional, measurable treatment goals in collaboration with patients and caregivers.
  • Develop individualized, age-appropriate plans of care with defined timelines and expected outcomes.
  • Educate patients and caregivers on treatment plans, goals, and rehabilitation expectations.
  • Collaborate with physicians and interdisciplinary team members to coordinate patient care.
  • Complete all required documentation, including initial evaluations, plans of care, and outcome measures.
Treatment Implementation
  • Deliver skilled physical therapy interventions, including therapeutic exercise, mobility training, and other evidence-based treatment techniques.
  • Explain treatment procedures and expectations clearly and professionally to patients and caregivers.
  • Monitor patient response to treatment and adjust interventions as clinically indicated.
  • Maintain compliance with safety protocols, infection prevention practices, and departmental standards.
  • Communicate patient progress and significant changes in condition to referring providers and care teams.
  • Develop and provide home exercise programs, discharge instructions, and caregiver education to support continued progress outside of therapy sessions.
Documentation & Compliance
  • Accurately document daily treatment sessions, patient progress, and outcomes.
  • Complete interim, weekly, and discharge documentation according to department and regulatory requirements.
  • Ensure all patient charges and records are accurately completed and submitted in a timely manner.
  • Record patient education activities and functional outcome measures as required.
  • Maintain compliance with all departmental, hospital, state, and federal documentation standards.
Professional Development & Team Engagement
  • Participate in departmental meetings, in-service education, and required hospital training.
  • Maintain current licensure, BLS certification, and all required competencies.
  • Pursue continuing education opportunities to support professional growth and clinical excellence.
  • Support orientation and training of new team members.
  • Supervise Physical Therapy and Physical Therapist Assistant students in accordance with Mercy and academic program guidelines.
