Anchor Health is seeking a Hospice Medical Social Worker (MSW) in San Joaquin County to support patients and families facing terminal illness. The MSW will provide psychosocial assessment, crisis intervention, care planning, and coordination with the hospice interdisciplinary team.
You will educate families on advanced directives, connect them with community resources, and help arrange funeral, financial, and healthcare decision planning as needed. Travel within the service area is required.
