Pain & Palliative Care NP​/PA | New Grads SNF

A growing pain management practice in Sacramento is hiring NPs and PAs to provide specialized pain and palliative care to geriatric residents in skilled nursing facilities. You'll round on the same residents weekly, manage pain medications, and build individualized treatment plans — meaningful, longitudinal work with an underserved population. New graduates welcome with formal onboarding. Malpractice with tail coverage included. Positions added as new facilities come on board.

• Conduct comprehensive pain assessments and physical exams
• Develop evidence-based treatment plans for acute and chronic pain
• Prescribe and manage pain medications per state and federal regulations
• Coordinate with physical therapy, acupuncture, and interventional services
• Educate patients and families on pain management and lifestyle strategies
• Monitor outcomes through follow-up visits and adjust treatment plans
• Document accurately in TEBRA and Point Click Care
• Build strong relationships with SNF leadership and staff

• Active, unrestricted California NP or PA license
• AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification
• Fully onsite; able to travel between Sacramento area facilities
• New graduates welcome; 5+ years relevant RN experience also counts
• Background in geriatrics, skilled nursing, palliative care, or pain management preferred
• TEBRA or Point Click Care  experience a plus

• $60–$100/hour + productivity bonus | ~$115,200–$192,000/year
• Full-time | ~32 hours/week | Schedule varies by facility
