Clinical Pharmacist
Location: Onsite for the first month of training in our Glendale (Los Angeles) office, with the opportunity to transition to a remote role thereafter

Our pharmacist-led clinical teams provide Medication Therapy Management (MTM) services that go beyond traditional pharmacy practice, working directly with patients to improve medication adherence, conduct comprehensive medication reviews, support chronic disease management, and identify and resolve barriers that may impact a patient’s health.
By combining clinical expertise with innovative technology and data-driven insights, our MTM Pharmacists help improve outcomes across HEDIS, Star Ratings, and other key quality measures—while building meaningful connections with patients and helping them achieve better health outcomes.
Key Responsibilities
- Conduct comprehensive medication reviews and other pharmacist-led clinical consultations with patients.
- Educate patients on their medications, disease states, adherence, and overall health.
- Identify medication-related problems, adherence barriers, and gaps in care and provide appropriate interventions.
- Collaborate with patients, pharmacies, prescribers, health plans, and other healthcare professionals to resolve medication-related concerns.
- Support patients with chronic disease management and help improve medication adherence and health outcomes.
- Complete accurate and thorough documentation of patient interactions and clinical interventions.
- Utilize technology and data-driven tools to identify patient needs and guide clinical conversations.
- Follow established clinical workflows, quality standards, HIPAA requirements, and program-specific guidelines.
- Meet established productivity and quality expectations while maintaining a high standard of patient care.
- Adapt to evolving clinical programs, technologies, and healthcare initiatives as Med Watchers continues to grow.
Requirements
- Doctor of Pharmacy (PharmD) degree or equivalent pharmacy degree from an accredited program.
- Ability to commit to a full-time, 40-hour workweek
. - Ability to work in a primarily desk-based, patient-facing role, including conducting telephone-based patient consultations.
- Reliable high-speed internet connection and a dedicated, professional workspace suitable for confidential patient conversations.
- Strong clinical knowledge, critical-thinking, and problem-solving skills.
- Comfortable utilizing technology and learning new healthcare platforms and systems.
- Ability to work independently in a remote environment while collaborating effectively with a multidisciplinary team.
