NP​/PA – Pain & Palliative SNF New Grads

Geriatric pain in skilled nursing facilities is one of the most undertreated areas in medicine. A growing pain management practice in Richmond and the East Bay is hiring NPs and PAs who want to be the provider that actually changes that. Round on the same residents weekly, build individualized pain treatment plans, and earn $60–$100/hour plus productivity bonuses with full-time consistent hours.

New graduates welcome. Malpractice with tail coverage included. New seats added as facilities expand.

• Conduct thorough pain assessments including history, physical exam, and identifying source and severity
• Build individualized treatment plans combining medication, non-drug, and interventional approaches
• Manage prescriptions responsibly per state and federal regulations
• Coordinate referrals for physical therapy, acupuncture, spinal blocks, and interventional care
• Guide patients and families through pain management strategies and lifestyle modifications
• Partner with physicians and facility providers to keep care aligned
• Track treatment effectiveness through follow-ups and adjust plans as needed
• Maintain accurate documentation in TEBRA and Point Click Care

• Active, unrestricted California NP or PA license
• AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification
• Fully onsite; comfortable traveling between East Bay facilities
• New graduates welcome; 5+ years relevant RN experience also counts
• Background in geriatrics, skilled nursing, palliative care, or pain management preferred
• TEBRA or Point Click Care  experience a plus

• $60–$100/hour + productivity bonus | ~$115,200–$192,000/year
• Full-time | Schedule varies by facility and caseload
