## Physical Therapist (PT) – Part-Time

Essential Duties and Responsibilities
* Evaluate residents’ mobility, strength, balance, coordination, range of motion, and functional abilities.
* Develop individualized treatment plans based on each resident’s needs, goals, and medical condition.
* Provide skilled physical therapy services to improve mobility, independence, safety, and overall function.
* Help residents regain the ability to walk, transfer, maintain balance, and complete daily activities safely.
* Establish measurable goals and modify treatment plans based on residents’ progress and changing needs.
* Provide therapeutic exercise, gait training, balance activities, transfer training, and other appropriate interventions.
* Recommend and train residents in the safe use of walkers, wheelchairs, canes, and other adaptive equipment.
* Educate residents, families, caregivers, and facility staff regarding treatment plans, safety, positioning, and mobility techniques.
* Complete evaluations, progress notes, discharge summaries, and other required documentation accurately and on time.
* Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary team.
* Participate in resident care conferences, discharge planning, and quality-improvement activities.
* Supervise Physical Therapist Assistants and rehabilitation support staff as permitted by law and facility policy.
* Maintain residents’ privacy and the confidentiality of all health information.
* Follow all professional standards, facility policies, safety procedures, and regulatory requirements.
* Perform additional duties as assigned.### Qualifications
* Graduate of an accredited physical therapy program.
* Current and unrestricted California Physical Therapist license required.
* Current CPR/BLS certification required.
* Skilled nursing, rehabilitation, long-term-care, or geriatric experience preferred.
* New graduates are encouraged to apply.
* Strong clinical assessment, communication, documentation, and time-management skills.
* Ability to work independently and collaboratively as part of an interdisciplinary team.
* Knowledge of Point Click Care  or another electronic medical record system preferred.
