Whitney Oaks Care Center is seeking a compassionate, skilled, and motivated Physical Therapist to join our dedicated rehabilitation team. • Evaluate residents and develop individualized treatment plans
• Deliver evidence-based physical therapy interventions
• Improve patients' mobility, strength, balance, and overall function
• Collaborate with physicians, nursing staff, therapy team members, and families
• Document evaluations, progress notes, and discharge plans accurately and timely
• Educate patients and caregivers on treatment plans and home exercise programs
• Participate in interdisciplinary care planning meetings

• Current California Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program
• Strong clinical assessment and treatment skills
• Excellent communication and interpersonal abilities
• Experience in skilled nursing, post-acute, rehabilitation, or long-term care preferred
• New graduates with strong clinical training are encouraged to apply
