PAY RANGE: $31.37 to $38.34/hr (depending on years of outreach experience)

LOCATION: Must be able to travel to different clinics and locations within Napa, Solano and Yolo county for outreach events. Home site TBD

SCHEDULE: 40 hours a week, and able to flex schedule for evenings and weekends to attend outreach events outside of normal business hours

Established in 2023, Communi Care+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and Communi Care Health Centers of Yolo County. JOB SUMMARY/OVERVIEW:

The Community Health and Outreach Coordinator is responsible for coordinating the activities of the Community Health Workers/Promotora team, overseeing outreach activities including, tabling, food distribution and large community events. The ideal candidate is skilled at establishing long-term partnerships and has experience developing and managing community health and outreach programs that will increase impact for those served by CCOLE. Candidates with experience working with Spanish speaking communities is preferred.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING

1. Bachelor’s Degree OR a combination of academic education, professional training or work experience, which demonstrates the ability to perform the duties of the position.
