Position Summary:
Responsible for providing social work services to clients of the Albert Pujols Wellness Center for Adults with Down Syndrome. Assesses, plans, and coordinates services to meet each individual's unique and complex needs by addressing biopsychosocial factors and available resources across the continuum of care. This position is also responsible for planning and implementing Wellness Center programs for clients and parents/caregivers, focusing on safety, overall wellness, social engagement, and quality of life.
Demonstrates an ongoing commitment to enhancing individual well-being and supporting community health. Responsible for promoting teamwork with all members of the healthcare team.
Performs all duties in a manner consistent with St. Luke’s mission and values.
Education, Experience, & Licensing Requirements:
Education: MSW in social work from a school accredited by the Council on Social Work Education
Experience: Experience working with individuals with Down Syndrome or other developmental disabilities, is strongly preferred.
