Behavioral Health Case Manager

Location: California

* Duties and Responsibilities:

*** 1. Ensure that patients receive appropriate quality care that is cost effective.
2. Works with patients to assess their conditions and needs to develop personalized care plans.
3. Conducts comprehensive assessments of clients' behavioral health needs
4. Develop individualized care plans based on client assessments
5. Coordinate and monitor implementation of care plans
6. Provide case management services, including referrals and coordination of services
7. Advocate for clients to ensure they receive appropriate and timely services
8. Maintain thorough and accurate client records/documentation and scans hardcopy medical documents into EMR system.
9. Work collaboratively with healthcare providers, social workers, and other professionals
10. Provide crisis intervention and support when necessary
11. Monitor client progress and adjust care plans as needed
12. Facilitate group therapy sessions and support groups
13. Educate clients and families about available resources and services
14. Ensure compliance with all applicable laws, regulations, and standards
15. Communicates with hospitals, pharmacies, health departments, clinics, insurance companies, government agencies, and health providers.
16. Coordinate referrals between Specialty Providers.
17. Performs other duties as assigned.
1. Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) certification
2. Familiarity with community resources and support services
3. Strong interpersonal and communication skills
4. Ability to handle high-stress situations effectively
5. Proficiency in electronic health records (EHR) systems
6. Minimum of 2 years of experience in a healthcare or social services setting
7. Knowledge of mental health and substance abuse disorders
8. Excellent assessment and analytical skills
9. Ability to develop and implement effective care plans
10. Strong organizational and time-management abilities
11. Demonstrates analytical and critical thinking abilities with pro-active decision-making and negotiation skills
12. Empathy and dedication to client advocacy
13. Must be able to work independently.## Qualifications

* Qualifications:

*** 1. Master’s degree in social work, Psychology, or related field
2. Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) certification
3. Current CPR Certification.
4. Experience in case management or behavioral health settings.
5. Certification in Case Management preferred.
6. Willingness to undergo and pass background checks and drug testing.

*** Typical Physical Demands:
*** Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination.  Requires standing and walking for prolonged periods of time.  Occasionally lifts and carries items weighing up to 50 pounds. Requires corrected vision and hearing to normal range.  Requires exposure to communicable diseases or bodily fluids.
