Assisted Living Waiver (ALW) Assessments

All Hours Adult Care (AHAC), a DHCS-listed Care Coordination Agency for California's Assisted Living Waiver (ALW) Program, is hiring a full-time, staff Registered Nurse to conduct ALW assessments for seniors and adults with disabilities across California. This is a staff position, not a contract role, paying $45.00–$55.00 per hour
, depending on experience.

As the RN, you will conduct ALW assessments, in-person and telephonic, for an assigned caseload of AHAC program participants. You will administer the DHCS-approved ALW Assessment Tool, gathering and documenting participant health and functional information to inform each participant's Individualized Service Plan (ISP) and the care-planning process.

• Administer the ALW Assessment Tool, gathering and documenting participant health and functional information to inform the Individualized Service Plan (ISP) and care-planning process.
• Submit completed assessments and required documentation through AHAC-approved systems and Med Compass, DHCS's approved system, within required time frames.
• Complete mandatory AHAC and DHCS training applicable to the role.
• Maintain timely, accurate recordkeeping and cooperate with AHAC's quality-assurance and DHCS audit processes.

• Current, unsuspended, and unrevoked license to practice as a Registered Nurse in the State of California.
• Minimum of 1,000 hours of experience in an acute-care setting,
  OR minimum of 2,000 hours of experience in a home setting(s), providing nursing care to patients with similar care needs.

• Familiarity with the Medi-Cal Assisted Living Waiver (ALW) program.
• Experience with Enhanced Care Management (ECM), CalAIM, or other community-based long-term-care programs for seniors and adults with disabilities, such as MSSP, CBAS, or PACE.
• Experience conducting functional assessments in a skilled nursing facility setting (RAI/MDS); RAC-CT is a plus.
• Experience in hospital discharge planning or case management for seniors or adults with disabilities; CCM is a plus.
• Experience conducting home health functional assessments (OASIS).
• Experience working with seniors and/or adults with disabilities in assisted living, residential care, or other community living settings.
• Experience with DHCS's Med Compass documentation platform.
• ASL proficiency is a plus.

• $45.00–$55.00 per hour, depending on experience.
• Full-time, approximately 40 hours per week, hybrid work arrangement.
