Innovate Rehab and Wellness is seeking a Licensed Physical Therapist (PT) to join a skilled nursing facility team. You will evaluate patients, identify movement limitations, and lead individualized treatment plans to restore movement and quality of life.
Work closely with an interdisciplinary team to optimize recovery and outcomes. Responsibilities include hands-on therapy, patient and caregiver education, progress tracking, and plan adjustments.
