Physical Therapist - Outpatient

Provides outpatient physical therapy evaluation and treatment in a 1:1 setting, working with a diverse patient population to improve function and reduce pain. Collaborates with patients, families, and healthcare team members, offering education, making referrals, and delegating tasks to assistants and students. Ensures accurate documentation and conducts assessments and reassessments of patient progress.

• Develops and implements individualized treatment plans considering discharge needs and equipment.
• Educates and consults with patients, families, and healthcare providers, making appropriate referrals.
• Delegates interventions to assistants and techs based on competency.
• Maintains proper documentation following departmental policies.
• Provides ongoing evaluation and re-evaluation of patient progress.

• Bachelor's Degree in Physical Therapy (required);
  Master's Degree preferred.
• Valid Physical Therapist license.
• No prior experience required; supervisory experience not necessary.
