As a Home Health Caregiver with Modani Care, your responsibilities will include:

• Providing personalized care services to clients in their homes, including assistance with daily living activities, medication management, and mobility support
• Maintaining accurate and detailed records of client care and services provided
• Collaborating with the care team to develop and implement personalized care plans that meet each client's unique needs
• Delivering exceptional customer service to clients and their families, addressing concerns and providing reassurance and support
• Maintaining a safe and clean environment for clients, adhering to all safety protocols and infection control measures

• Ability to lift and transfer clients safely and assist with mobility needs
• Effective communication and interpersonal skills
• Ability to work independently and as part of a team
• Reliable transportation and ability to travel to client homes
