RNIII Triage NursePart Time
Salary Range: $61.00 To $64.00 Hourly
Hospice of the East Bay (HEB) is a community-based program of home care for terminally ill patients. Care is given to patients and families by an interdisciplinary group. Bereavement care is provided to the family for one year after the death. Hospice services are given regardless of ability to pay, making the agency dependent on donations and insurance billing to meet operating expenses.
POSITIONSUMMARY
Reporting to the RN Team 4 Supervisor, this position is responsible for routing calls to appropriate field staff, scheduling staff as appropriate, ensuring staff has been appropriately scheduled and that emergent needs are identified and staff is dispatched.
ESSENTIAL FUNCTIONS- Receive handoff from Friday scheduler and handoff to Monday scheduler via email or Teams. Saturday and Sunday hand off to the PM shift and Administrator on call.
- Receive daytime phone calls from patients/families/triage service regarding changes in status or the plan of care.
- Arrange for any necessary visits or continuous care shifts.
- Attend and participate in nursing and agency staff meetings.
- Coordinate care with other agencies, suppliers and facilities participating in patient care as necessary.
- Maintain necessary records and provide requested reports in a timely manner.
- Review Hospice on Call documentation, as requested, to ensure it meets all State, Federal and agency requirements; educate staff as necessary regarding documentation requirements.
- Attend all appropriate in-services given for HEB staff.
- Participate in quality management and performance improvement activities as requested.
- Provide the required LVN oversight and direct oversight of RN/Spiritual Care and Social Services.
- Provide direct patient care if needed.
- Maintain on-site presence during scheduled hours.
- Demonstrates excellent interpersonal and communication skills, initiative, dependability and good judgment.
- Knowledge and understanding of hospice philosophy and hospice criteria for services under the Hospice Benefit.
- Knowledge of State, Federal regulations and accreditation standards.
- Demonstrates excellent skills in standards of practice for hospice care including pain and symptom management.
- Must be able to work well under pressure meeting multiple priorities and sometimes competing deadlines.
- Demonstrates strong organizational, time management and development skills.
- Demonstrates, at all times, cooperative behavior with colleagues and management staff.
- Ability to assess and respond to needs of patients, families and caregivers via phone communication and ensure appropriate field staff are dispatched or are aware.
- Ability to communicate effectively with family and assist with coping and emotional stress; is tolerant of individual lifestyles and is nonjudgmental.
- Demonstrates ability to work collaboratively as part of an interdisciplinary team as well as independently, and problem solve in a crisis situation.
- Demonstrates clinical competency as observed during supervisory visit.
- Current California Registered Nurse License required.
- At least two years’ experience working as a professional nurse caring for the terminally ill.
- CPR certification required.
- BSN or MSN preferred.
- Certification as Hospice and Palliative Care Nurse preferred.
