We make house calls – enabling access to quality wrap‑around services for homebound older adults with complex health needs while improving the quality of their lives, right offer an array of medical and chronic care services and we’re backed by SCAN, a non‑profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.
Homebase Medical is expanding rapidly into the Woodland Hills / San Fernando Valley, CA area.
- Conduct Annual Wellness Visits focused on patient education
- Deliver Chronic Care Management to improve long‑term outcomes
- Perform comprehensive in‑home visits, including medication review, chronic illness management, medical history updates, physical exams, labs, diagnostic screenings, and post‑hospital follow‑ups
- Provide primary‑care management and palliative care services
- Active Nurse Practitioner or Physician Assistant license in the State of California
- Board certification (Nurse Practitioner or Physician Assistant)
- DEA licensure
- Prior experience preferred (but passion and commitment are just as important)
- Pay: $65.00 – $90.00 per hour
