This position is a hybrid opportunity in Riverside, requiring 60% on‑site patient care and 40% case management.

• Perform comprehensive health assessments, including history taking and physical examination.
• Complete review of medicines and related instructions.
• Review home safety and psychosocial needs.
• Fulfill documentation needs for STAR and HCC measures.
• Obtain and document all pertinent information in patient charts and case management notes on the same day as the visit.
• Include medications and end‑of‑life preferences and drivers of preventable hospital admissions.
• Instruct patients, caregivers, and families to call you prior to accessing medical care, subject to life‑threatening emergencies.
• Maintain patient safety and confidentiality using universal precautions.
• Prescribe or order procedures, treatments, and medications within scope of practice.
• Develop a high level of trust and intimacy with patients and families.

• CA Licensed Nurse Practitioner.
• 1+ years of experience in primary care.

Base Salary: $120,000 - $150,000 per year. Additional compensation may be awarded based on education, experience, and qualifications.
