Overview
The Licensed Practical Nurse (LPN) Primary Care PACT serves veterans in an ambulatory setting at VA St Louis Health System sites including John Cochran Campus, Jefferson Barracks, Annex Clinic of Washington Avenue, Manchester Avenue, and Community Based Outpatient Clinics in Franklin County, St Charles County, St Louis County in Missouri, St Clair County, and Scott Air Force Base in Illinois.
Responsibilities- Provide nursing care within the LPN scope of practice, working under the supervision of an RN/MD/DO and in coordination with the care team to promote patient safety and quality health care.
- Observe and document patient responses to medications and treatments, report changes to the RN or MD/DO, and participate in updating patient-driven holistic care plans.
- Initiate appropriate actions based on the treatment plan and patient condition, and assist in resuscitation procedures as directed by the RN/MD/DO.
- Collaborate with the patient care team (Provider, RN Care Manager, Nursing Assistants, Health Technicians) to orient/train less experienced staff and ensure adherence to established standards of practice.
- Educate patients and/or family members regarding common disease processes, medications, and prescribed treatment regimens.
- Prepare for and administer prescribed medications (oral, topical, subcutaneous, intramuscular, and/or intravenous as allowed by policy) and document actions taken and patient responses.
- Recognize and respond to equipment issues, safety hazards, and supply deficiencies; report for corrective action as needed.
- Act as a preceptor or mentor to less experienced LPNs/LVNs or NAs/HTs as related to support duties for complex procedures.
- Engage in ongoing learning and share new knowledge with the team to improve nursing practice.
- Work schedule:
Rotating schedule.
Basic Requirements:
- United States Citizenship (non-citizens may be appointed only as allowed by VA policy)
- English Language Proficiency
- In accordance with 38 U.S.C. 7402(d), proficient in basic written and spoken English for direct patient care roles
- Education:
Graduate of a school of practical or vocational nursing approved by the state or NLNAC-accredited at the time of program completion - Licensure:
Active, full, current, and unrestricted licensure as a Graduate LPN/LVN in a U.S. state, territory, commonwealth, or District of Columbia - Grading/Experience requirements by grade level (GS-03 to GS-04 to GS-05) including demonstrated nursing competencies and ability to provide care to a variety of patient populations; collaborate with RNs/MDs; educate patients; document care; and respond to emergencies
