Provide direct patient care based on RN and physician care plans

Administer medications, injections, wound care, and treatments

Monitor vital signs and report changes in patient condition

Maintain accurate and timely clinical documentation

Support patients with personal care within LVN scope

Promote infection control and patient safety

Communicate effectively with care team and family members

Graduate of an accredited nursing program

Current CPR certification

At least 1 year of home health or private duty nursing experience
