Community Registered Nurse (RN)Part‑Time
Position SummaryThe Community RN will support the CHS mission by promoting the physical and mental health of individuals through application of nursing processes, and by ensuring qualified staff through the delivery of Level 1 Medication Administration and CPR/First Aid instruction.
Essential Duties and Responsibilities- Promote physical and mental health of individuals by coordinating medical services and monitoring medication in accordance with regulatory requirements. Provide at least 1.25 hours of service to each individual supported each month; participate in admissions; clarify medical information; advocate in hospital admissions/discharges; communicate with healthcare providers or pharmacists to review health statuses and medication plans; conduct full health assessments within 7 days of discharge from hospitalization or ER visits;
coordinate ongoing care with clinics, external providers, hospitals, and pharmacies; review and teach health and nutrition per individual needs; conduct professional assessment and monitoring services as authorized; promote self‑medication and self‑care; participate in on‑call rotations with other community nursing staff as directed. - Ensure medically qualified staff by providing nursing delegations related to diagnoses, allergies, medication, diet, and emergency plans; become certified to instruct Level 1 Medication Administration and CPR/First Aid training; develop instruction methods and teaching aids; conduct in‑person, recorded, or virtual training via appropriate methods; test trainees to measure progress; report progress of employees in training to departmental management; supervise Community LPN services when required.
- Communicate complex medical information to individuals served, staff, and stakeholders; develop plans to meet medical needs; follow up on health concerns identified by the Department of Mental Health RN; complete the medical sections of Individual Support Plans; participate in departmental and staff meetings for alignment and knowledge transfer; answer health‑care questions from staff, guardians, coordinators, and individuals.
- Support regulatory compliance by documenting services per agency policy and Medicaid, DMH certification, and other contractual needs; review medication documents; complete CIMOR and APTS 2 referrals timely; input information into the state HRST system and update monthly; maintain knowledge of company policies; protect confidentiality per HIPAA; maintain licensing and certifications; pursue continuing education; act as local medical advisor for infectious disease control and health crises;
serve as infectious disease control authority when directed by the CEO or designee. - Promote a positive work culture by supporting other nurses with teamwork, coverage for absences, willingness to carry out special assignments; respect individuals’ rights; advocate on their behalf; follow CHS policies; contribute to medical policy maintenance; present a positive attitude; respect differences and demonstrate cultural competence; communicate professionally; project a positive image of CHS and individuals served.
