Job Overview

The Advanced Practice Clinician will provide and coordinate primary care to SCAN members, ensuring timely, appropriate care that promotes independence and reduces hospital utilization.

Responsibilities
  • Conduct physical/wellness exams and assessments in patients’ residences, senior living communities, or skilled nursing facilities.
  • Perform urgent medical visits, telephonic triage, and administer vaccines as needed.
  • Order and interpret diagnostic tests and manage chronic diseases.
  • Initiate goals‑of‑care conversations with patients, families, and community staff.
  • Collaborate with case management to secure timely access to services.
  • Maintain a visitation schedule that meets practice protocols and adapts to patient acuity.
  • Engage with Primary Care Physicians and other health professionals to promote disease prevention, health promotion, and chronic disease management.
  • Partner with specialists, hospitals, ancillary providers, and community organizations to ensure continuity and shared decision‑making.
  • Monitor and address clinical quality gaps in partnership with other care providers.
  • Document all visits and interactions promptly in accordance with protocols.
  • Act as a direct resource for members, families, and support staff.
  • Participate in Integrated Care Sales team events to raise member awareness and educate communities.
  • Initiate and conduct virtual health visits to improve triage accuracy and reduce travel time.
  • Participate in the on‑call program for after‑hours and weekend care.
  • Maintain professional and technical knowledge through workshops, publications, and continuing education.
  • Contribute to team efforts to meet quality healthcare goals by closing care gaps, attending high‑risk group discussions, and optimizing workflows.
  • Other duties as assigned.
Qualifications
  • Master’s degree in Nurse Practitioner Program, PA program, or equivalent combination of education and experience.
  • Current California Nurse Practitioner license or California Physician Assistant (PA‑C) certification in good standing.
  • Wound Care Certificate and/or Palliative Care Certificate preferred.
  • Knowledge of complex chronic disease management (dementia, diabetes, heart failure, COPD).
  • Ability to identify health care needs, make referrals, and coordinate resources appropriately.
  • Familiarity with HCC and ICD‑10 coding and documentation.
  • Knowledge of CMS guidelines and Medicare Advantage Managed Care.
  • Comfortable initiating goals‑of‑care and end‑of‑life discussions.
  • Strong interpersonal skills and a change‑agent mindset for building relationships.
  • Understanding of HIPAA requirements and ability to maintain confidentiality.
  • Adaptable, open to change, and skilled in problem‑solving.
  • Basic technological proficiency (Word, Excel, PowerPoint, Outlook, EMR) and openness to new technology.
  • Physical ability to stoop, kneel, crouch, bend, reach, lift, pull, push, and carry up to 10lbs regularly and 50lbs occasionally.
  • Multilingual/multicultural background a plus.
  • Travel 75%+ of the time in the assigned area; must hold a valid driver’s license, automobile insurance, and reliable transportation.
