We are seeking a Registered Nurse to perform multidisciplinary communication and facilitation of care throughout a patient’s hospitalization. The role includes reviewing hospitalizations, determining medically necessary levels of care, collaborating with the multidisciplinary team and payers, and developing individualized discharge plans.
Responsibilities- Perform pre‑admission, admission, and continued stay reviews per CMS guidelines, commercial payer guidelines, and hospital policy.
- Recommend a level of care that meets medically necessary hospital care criteria and confirm clear, specific level of care orders and documentation.
- Act as liaison between ED physicians, hospitalists, attending physicians, physician advisors, post‑acute and third‑party payers, and patients/caregivers.
- Collaborate with the care team to develop and implement discharge plans based on patient‑specific needs.
- Maintain open communication with all appropriate parties and keep leadership informed of functional and financial outcomes.
- Ensure compliance with all applicable federal, state, local laws and accreditation standards related to utilization review and discharge planning.
- Perform other related duties as required.
- Graduate of an accredited school of nursing.
Education and Experience
- Bachelor’s degree in nursing.
- Three (3) years of hospital‑based experience in discharge planning, case management, or utilization review.
- Current registered nurse license in the state of practice.
- Basic Life Support (BLS) from the American Heart Association.
- Certification in Case Management (CCM) or Accredited Case Manager (ACM).
- Proficiency in using computers, software, and web‑based applications.
- Effective verbal and written communication skills with ability to present information clearly to varying audiences.
- Strong interpersonal skills.
- Ability to work a flexible schedule, including 24/7, weekends, holidays, and on‑call availability.
Medium work requiring occasional lifting of 20–50 pounds and frequent lifting of 10–25 pounds. Routine duties involve exposure to blood, body fluids, tissues, and potentially hazardous medications or waste. Work is performed in patient care areas with occupational risk for communicable disease exposure.
