• Facilitates identification and resolution of gaps in care from hospitalization through discharge and into ambulatory settings.
• Critically assesses patient data to proactively mitigate impending changes in condition and establishes consults to interprofessional teams as appropriate.
• Leads development and implementation of complex individualized care plans with defined goals in collaboration with family, caregivers, and interprofessional team members.
• Utilizes and champions evidence‑based practices through participation in journal clubs, literature reviews, and dissemination of new knowledge.
• Explores opportunities to advance nursing practice, including community outreach, ethics advisory participation, adjunct faculty roles, and specialty organization leadership.
• Analyzes gaps in practice standards and collaborates with leadership to enhance safe patient care.
• Leads evaluation of patient and family experience and adjusts care to improve satisfaction and quality.

• Education:
  
  Nursing Diploma or Associate's Degree in Nursing.
• Experience:
  
  2–5 years of nursing experience.
