$3,500 Sign‑on Bonus for External Candidates

Optum Home & Community Care, part of the Optum family, is creating a new integrated care model that responds to patients’ physical, mental, and social needs. As a member of our Senior Community Care (SCC) team, you will provide care to patients in nursing homes, senior housing, and assisted living settings, delivering comprehensive assessments, documenting findings, and participating in care coordination.

The Patient Connect program is a one‑time comprehensive visit for patients who have recently been hospitalized and are in a skilled rehabilitation facility. The purpose is to ensure a smooth transition from rehab to home and to prevent readmission. In this role you will capture STAR measures, educate the patient, and document all acute and chronic diagnoses. You will communicate findings to attending physicians and facility staff but will not be responsible for orders, treatments, or follow‑up.

You will work independently and exercise the autonomy accorded to this position.

Primary Responsibilities
  • Perform comprehensive assessments and document findings in a concise manner that complies with documentation requirements and CMS regulations.
  • Ensure that all diagnoses are ICD‑10 coded accurately and documented appropriately.
  • Ensure that all quality elements are addressed and documented.
  • Conduct an initial medication review.
  • Attend and complete all mandatory educational and computer/virtual training requirements.
  • Travel between care sites as required.
  • Care coordination: communicate with the patient and the necessary team members regarding any medical concerns during your visit.
  • Program Enhancement Expected Behaviors:
    • Exhibit original thinking and creativity in developing new and improved methods and approaches to concerns or issues.
    • Function independently and responsibly with minimal need for supervision.
    • Enter available hours into the web‑based application at least one month prior to available work time.
    • Demonstrate initiative in achieving individual, team, and organizational goals and objectives.
    • Participate in SCC quality initiatives.
    • Check Optum email intermittently for required trainings, communications, and monthly scheduling.
Required Qualifications
  • Active and unrestricted Nurse Practitioner license in Missouri.
  • Certified Nurse Practitioner through a national board: graduate of an accredited master’s degree in nursing (MSN) program and board‑certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), Adult‑Gerontology Acute Care Nurse Practitioner (AG‑AC NP), Adult/Family or Gerontology Nurse Practitioner (ACNP), with preferred certification as ANP, FNP, or GNP.
  • Ability to move a 30‑pound bag in and out of a car and navigate stairs and a variety of dwelling conditions and configurations.
  • Availability to work 10 hours per week.
  • Access to reliable transportation that allows travel to client or patient sites within the designated area.
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records.
Preferred Qualifications
  • At least one year of nursing home experience.
  • Experience meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of geriatrics and chronic illness.
  • Understanding of advanced illness and end‑of‑life discussions.

Compensation for this specialty generally ranges from $90 to $155 per hour. Total cash compensation includes base pay and bonus and is based on several factors, including local labor markets, education, work experience, and may increase over time based on productivity and performance in the role. In addition to salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (all benefits are subject to eligibility requirements).
