Job Description Summary

Responsible for managing patient care and treatment through in‑home clinical visits, in close collaboration with the physician and interdisciplinary care team. • Conduct in‑home health assessments and focused physical exams
• Manage acute and post‑acute conditions for patients transitioning from hospital, ER, or nursing facilities
• Develop, implement, and document individualized care plans
• Order, interpret, and evaluate diagnostic tests
• Prescribe medications and treatments within scope of practice
• Collaborate closely with physicians, care managers, and interdisciplinary team members
• Educate patients and families to support recovery, safety, and long‑term health at home
• Maintain timely, accurate clinical documentation

• Master’s degree in Nursing (MSN) with Adult/Geriatric Nurse Practitioner certification
• Minimum 1 year of Nurse Practitioner clinical experience (home‑based or transitional care experience a plus)
• Active, unrestricted RN and NP license in the applicable state
• Current TB test or willingness to obtain prior to start

Full‑time, Monday–Friday. Salary Range: $ – $ annually, with bonus eligibility based on individual and company performance.

This position serves patients throughout the South Bay, including:
Lawndale, Gardena, Compton, Torrance, Long Beach, Redondo Beach, Hawthorne.
