** Your Impact & Responsibilities:**
* ** Bedside Excellence:
** Provide high-quality nursing care, including medication administration, wound treatments, and resident assessments.
* ** Clinical Monitoring:
** Closely monitor resident vitals and conditions, promptly reporting any changes to the RN or physician.
* ** Care Coordination:
** Collaborate with our in-house therapy team (PT/OT/SLP) and physicians to ensure comprehensive recovery plans.
* ** Documentation:
** Maintain precise, timely, and compliant electronic medical records (Point Click Care  experience is highly valued).
* ** Leadership:
** Provide direction and mentorship to CNAs to ensure resident ADLs are met with the highest standards of care.
* ** Advocacy:
** Act as a compassionate resource for residents and their families during their transitional care journey.

* Qualifications:

**
* ** License:
** Must possess a current, valid
** California Licensed Vocational Nurse (LVN)
** license.
* ** Certification:
** Current CPR/BLS Certification.

* Experience:

** Previous experience in subacute, skilled nursing, or transitional care is preferred.
* ** Skillset:
** Strong clinical judgment, excellent communication skills, and a "team-first" attitude.
