Licensed Practical Nurse (LPN) 12-Hour Day Shift (Rotating Tue/Fri Schedule)

ResponsibilitiesProvides basic nursing care by assessing residents on admission and ongoing basis, communicates significant changes to interdisciplinary team (IDT), documents assessments accurately, seeks guidance when needed, participates and updates care plans, implements nursing and physician orders, administers medication safely, uses critical thinking for resident changes, notifies physicians using SBARR, maintains composure in crises, monitors care delivery, identifies problems and uses QAPI, identifies behavioral changes, responds to call lights, assists with ADLs, completes at least twelve hours of nursing continuing education, stays current with best practices, and may be assigned additional duties.
Qualifications- Education from an accredited nursing school leading to licensed practical nurse
- Current LPN license in the state of employment
- CPR and AED certification required, or willing to obtain within six months of employment
- IV certification required, or willing to obtain within six months of employment
- Long‑term care experience preferred
- Ability to assess basic nursing needs of geriatric residents
- Excellent communication skills to interact with residents, visitors and staff
- Time‑management and organizational skills
- Ability to stand and walk for long periods
- Ability to lift heavy objects with proper technique and assistance when needed
- Ability to transfer and move patients weighing 200+ lbs with proper technique and assistance
- Ability to push and pull equipment weighing in excess of 40 lbs with proper technique and assistance
- Manual dexterity, coordination and skillful use of hands
- Visual acuity with color discrimination
- Ability to perform necessary documentation electronically and in writing
$26.96 – $43.25 per hour
