• Active Kentucky RN license
• Minimum 2 years of recent experience as a Home Health Registered Nurse
• Proficient in OASIS and HCHB (Homecare Homebase) documentation systems
• Current CPR certification
• Valid driver’s license, vehicle insurance, and reliable transportation

• Provide patient‑centered nursing care in home health settings
• Perform comprehensive patient assessments and develop individualized care plans
• Document patient care accurately using OASIS and HCHB systems
• Collaborate with interdisciplinary healthcare teams for continuity of care
• Educate patients and families on health management, preventive care, and chronic disease management
• Travel within the La Grange, KY area to deliver clinical support to patients
