Job Overview

Signify Health, part of CVS Health®, is seeking board‑certified nurse practitioner contractors to conduct in‑home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

These evaluations focus on a person’s overall health and well‑being, identifying potential barriers to care. You will perform medication and medical history reviews and a physical evaluation in a member’s home. You are not required to prescribe medication, order laboratory tests, or alter existing treatment plans.

• Conduct in‑home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members
• Perform medication, medical history, and physical examinations
• Document findings using pre‑populated, patient‑specific forms
• Provide care within the scope specified by the health plan member’s provider, without prescribing or ordering tests

Shifts available Monday through Sunday. Nights and weekends are not required. Positions are short‑term or long‑term based on availability.

Base rate of $80 per completed assessment, with an average of 6‑9 visits daily. Compensation can increase up to 135% or 160% depending on the county through a Market Multiplier. Additional pay is available via a pay‑per‑test model for qualifying diagnostic and preventive tests. Weekly earnings can range from $2,500 to $4,000.
