• Our openings include full‑time, part‑time, and PRN for all shifts

• Provide patient care to residents
• Comply with state and federal regulation
• Work collaboratively with members of the healthcare team.
• Serve as a primary coordinator of all disciplines for well‑coordinated patient care.
• Assess and coordinate patient’s discharge planning needs with members of the healthcare team.

• Valid state license(s)
• Excellent verbal communication skills and strong interpersonal skills
• High integrity and well‑grounded work ethic
• Agility and flexibility with changing priorities
• Team‑oriented and a practical team‑player
• Professional demeanor
• At least one year experience providing care and services in long‑term care (preferred)
