FULL‑TIME (1.0) NOC SHIFT
APPROXIMATE PAY RANGE: $55.26‑$64.13
The Registered Nurse provides direct care in the Skilled‑subacute unit. The nurse communicates with physicians, the Nurse Director, and co‑workers about changes in the patient’s clinical condition, results of diagnostic studies, and symptomatology. The nurse responds quickly and accurately to changes in condition or treatment response.
Responsibilities- Consistently uses two patient identifiers when drawing labs, administering medications, and performing procedures.
- Does not use unacceptable abbreviations in documentation (uses read back on all telephone orders).
- Consistently follows medication reconciliation policy.
- Ensures the crash cart’s defibrillation function has been checked per protocol and that after each code the appropriate items are replaced and documented on the crash cart checklist.
- Complies with the Infection Manual (OSHA blood borne pathogen standards, CDC guidelines) and demonstrates appropriate use of personal protective equipment (gloves, gowns, masks, goggles) and hand‑washing techniques.
- Provides a thorough, detailed change‑of‑shift report to ensure consistency in the plan of care.
- Assists physicians with special tests and procedures in the unit and uses a time‑out on all patients for any bedside procedures.
- Avoids and prevents injury to self and patients by applying the principles of body mechanics; maintains a safe environment with knowledge of safety issues (e.g., suction and oxygen immediately available when needed).
- Conducts pain assessment and reassessment at appropriate intervals after pain medication and upon discharge or transfer of patients.
- Completes documentation of medications administered in the MAR and narcotic sheet/Pyxis.
- Recognizes signs of abuse and reports appropriately to Adult Protective Services.
- Initiates discharge and patient/family education planning with proper documentation on the patient record in Medi‑Tech.
- Establishes priorities of patient care based on essential needs and available resources (time, equipment, personnel, supplies).
- Formulates, maintains, and updates a goal‑directed plan of care on patients that is prioritized and based on patient outcomes.
- Performs other duties as assigned or required.

Required Qualification- Must have a current and valid license as a Registered Nurse issued by the State of California.
- Must hold BCLS upon hire and maintain it.
- Previous six months of experience in acute‑care hospital or long‑term care.
