Job Description:

Licensed Practical Nurse (LPN) – Skilled Nursing Facility (SNF)$7,000 SIGN ON BONUS Position Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide quality nursing care to residents in a skilled nursing facility. The LPN works under the supervision of a Registered Nurse (RN) or physician to support resident health, administer treatments and medications, and ensure comfort and safety in a long-term care and rehabilitation setting.

Key Responsibilities Provide direct nursing care to residents according to care plans

Administer medications and treatments as prescribed

Monitor and document residents’ vital signs, conditions, and responses to treatment

Report changes in resident condition to the RN, physician, or healthcare team Assist with admissions, discharges, and transfers

Supervise and support Certified Nursing Assistants (CNAs)
Maintain accurate and timely medical documentation in electronic medical records (EMR)
Ensure compliance with infection control, safety, and regulatory guidelines

Assist residents with activities of daily living as needed

Communicate effectively with residents, families, physicians, and interdisciplinary staff

Respond appropriately to emergencies and resident care needs

Qualifications

Current Licensed Practical Nurse (LPN) license in the state of employment

Graduate of an accredited practical nursing program

Current CPR/BLS certification

Previous experience in skilled nursing, long-term care, rehabilitation, or geriatrics preferred

Strong communication and organizational skills

Ability to work independently and collaboratively within a healthcare team Compassionate and resident-centered approach to care

Physical Requirements Ability to stand, walk, bend, lift, and assist residents throughout the shift

Ability to lift up to 50 pounds with or without assistance

Ability to respond quickly in emergency situations

Schedule & Work Environment Full-time, part-time, and PRN opportunities available

Day, evening, overnight, weekend, and holiday shifts may be required
