Registered Nurse (RN) Case Manager
Seeking an experienced and compassionate Registered Nurse (RN) Case Manager to provide exceptional hospice care to patients throughout the Hannibal and Northern Missouri territories. This role is responsible for managing an assigned hospice caseload, coordinating care with the interdisciplinary team, supporting hospice admissions, and ensuring patients and families receive high-quality end-of-life care. The ideal candidate will have hospice experience, Homecare Homebase (HCHB) knowledge, and a passion for delivering patient-centered care.

Coverage Area:
Seven-county radius, including Hannibal and Northern Missouri territories
Office Requirement:
Travel to the office at least once per week
Schedule:
Day shift with participation in on-call and weekend rotations
Weekly Hour Guarantee: 40 hours
Pay Rate: $51.00 - $53.00 per hour
Productivity Expectation: 25 points per week
Caseload:
Approximately 12-14 patients as primary case manager
Additional Responsibilities:
Assist with hospice admissions as scheduling permits

- Serve as the primary RN Case Manager for an assigned hospice patient caseload.
- Provide skilled nursing care and symptom management to hospice patients in their homes and care facilities.
- Conduct comprehensive patient assessments and ongoing evaluations.
- Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
- Coordinate care with social workers, chaplains, hospice aides, physicians, and other healthcare professionals.
- Manage a caseload of approximately 12-14 hospice patients while ensuring timely and appropriate service delivery.
- Assist with hospice admissions, assessments, and care transitions as scheduling allows.
- Educate patients and families regarding disease progression, symptom management, medications, and end-of-life care.
- Maintain accurate, timely, and compliant clinical documentation within agency systems.
- Participate in interdisciplinary team meetings and collaborate to achieve optimal patient outcomes.
- Respond to patient and family needs during on-call and weekend coverage assignments.
- Advocate for patient comfort, dignity, and quality of life throughout the hospice journey.
- Active Registered Nurse (RN) license.
- Previous hospice nursing experience required.
- Working knowledge and experience with Homecare Homebase (HCHB).
- Strong clinical assessment and case management skills.
- Excellent communication, interpersonal, and patient education abilities.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Ability to work independently and manage a geographically dispersed caseload.
- Strong organizational, time management, and documentation skills.
- Extensive hospice case management experience.
- Experience completing hospice admissions and start-of-care assessments.
- Strong understanding of hospice regulations, compliance standards, and best practices.
- Ability to effectively collaborate within an interdisciplinary team.
- Demonstrated commitment to compassionate, patient-centered end-of-life care.
- Scrubs or professional clinical attire permitted.
- No specific scrub color requirement.
