Salary Description
$33 - $37 / per hour

• A valid, current nursing license in Illinois.
• Current CPR certification.
• At least one year experience in long term care.

$30-32/hour | 8-Hour Shifts! Full Time 6AM - 2PM OR Part Time 2PM - 10PM | Weekend Availability Required

We are seeking a compassionate, dependable, and clinically strong LPN who is passionate about senior care and thrives in a team-oriented environment focused on dignity, quality of life, and resident-centered care.

Responsibilities:

• Provide nursing care and clinical support to residents
• Assist with resident assessments and care planning processes
• Administer medications and treatments according to physician orders and company policies
• Monitor resident conditions and communicate changes appropriately
• Maintain accurate clinical documentation and medication records
• Partner with caregivers, medication technicians, physicians, and families to ensure quality resident care
• Support resident dignity, comfort, and quality of life
• Assist with compliance, infection control, and quality assurance processes
• Support and mentor caregiving staff as needed
• Foster positive relationships with residents, families, and team members

Qualifications:

• Active LPN / Licensed Practical Nurse license in good standing
• Assisted living, memory care, long-term care, or senior living experience preferred
• Strong medication management and clinical assessment skills
• Excellent communication and teamwork abilities
• Compassionate and resident-centered approach to care
• Ability to work in a fast-paced environment while maintaining attention to detail
• Strong organizational and documentation skills

• Leads with compassion, professionalism, and accountability
