Mobile Nurse Practitioner (CareMore - San Gabriel Valley - East LA, CA)

Care More Health is a field‑based role requiring travel to various clinical sites and patient locations across the San Gabriel Valley and East Los Angeles service areas. Nurses will provide primary, acute and/or specialty healthcare services for patients in home or virtual settings.

• Provide a range of primary, acute and/or specialty healthcare services for patients in home or virtual settings.
• Where state requirements do not impose collaborative agreements, act as primary care provider.
• Order, interpret and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs.
• Provide health instruction, counseling and education on disease prevention and healthy lifestyle choices.
• Collaborate with physicians, nurses and other healthcare professionals to treat patient needs as part of an interdisciplinary care team.
• Formulate and document care plans; prescribe medication or other treatments.
• Participate in an on‑call program, which may include after‑hours, weekend and holiday calls and visitations.

• San Gabriel Valley:
  Pasadena, West Covina, El Monte
• East LA:
  Whittier, Pico Rivera, Downey

• MS in Nursing.
• Minimum of 1 year of related clinical experience for a nurse practitioner or equivalent combination of education and experience.
• Current, active, valid and unrestricted RN license and NP license in Nevada.
• Successful completion of a tuberculosis test.
