Job Description:

Transport Driver – Skilled Nursing Facility (SNF)

Job Summary:

The Transport Driver is responsible for safely transporting residents to and from medical appointments, therapy sessions, hospital visits, and other approved destinations while ensuring resident safety, comfort, and dignity.

Key Responsibilities:

Safely transport residents using facility vehicles.

Assist residents with entering and exiting vehicles, including wheelchair securement.

Follow all traffic laws and facility transportation policies.

Perform pre- and post-trip vehicle inspections and report maintenance needs.

Coordinate transportation schedules with nursing staff and appointment providers.

Maintain accurate transportation logs and mileage records.

Communicate changes in resident condition to nursing staff.

Keep vehicles clean, fueled, and ready for use.

Provide courteous, respectful, and compassionate service to residents.

Qualifications:

Valid driver's license (Class E or higher) with a clean driving record.

CPR certification (preferred or required by facility).Ability to safely assist residents with mobility needs.

Excellent communication and customer service skills.

Experience working with elderly or disabled individuals preferred.

Certified Nursing Assistant (CNA) – Skilled Nursing Facility (SNF)

Job Summary:

The Certified Nursing Assistant (CNA) provides direct patient care to residents under the supervision of a licensed nurse, promoting comfort, dignity, safety, and independence.

Key Responsibilities:

Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding.

Measure and record vital signs, weight, intake, and output as assigned.

Assist with transferring, repositioning, and ambulating residents using proper techniques.

Observe residents for changes in condition and report concerns promptly to the nurse.

Maintain clean, safe, and organized resident rooms.

Assist with meal service and hydration.

Document care accurately according to facility policies.

Follow infection control, safety, and HIPAA guidelines.

Provide emotional support and compassionate care to residents and families.

Qualifications:

Current CNA certification in the applicable state.

CPR certification (required).Ability to lift, transfer, and assist residents safely.

Strong communication, teamwork, and interpersonal skills.

Previous experience in long-term care or skilled nursing is preferred but not always required.
