About the Role:

As a Registered Nurse (RN) at Hermitage Nursing & Rehab, you will be responsible for providing high-quality nursing care to our residents, under the supervision of our experienced medical team. Your duties will include:

  • Assessing and evaluating the physical, emotional, and psycho-social needs of our residents

  • Developing and implementing individualized care plans to meet these needs

  • Administering medications, treatments, and therapies as prescribed by physicians

  • Monitoring resident conditions and reporting changes to the healthcare team

  • Educating residents and their families on disease management, self-care, and other health-related topics

  • Collaborating with other healthcare professionals to ensure seamless care coordination

Responsibilities:
  • Provide direct patient care to residents in our long-term care facility

  • Conduct initial and ongoing assessments of resident needs and develop individualized care plans

  • Administer medications, treatments, and therapies as prescribed by physicians

  • Communicate effectively with residents, families, and healthcare teams to ensure optimal care

  • Participate in quality improvement initiatives and maintain accurate documentation of resident care

  • Maintain a clean and organized environment, adhering to infection control and safety protocols
